IMPORTANT INFORMATION
PLEASE READ BEFORE BEGINNING THE 2009 RENEWAL SURVEY
Changes have been made to the “on-line” 2009 Renewal Underwriting Survey in an effort to make the renewal process easier.
We would like to again offer the following recommendations and ask that your 2009 Survey be submitted no later than September 15, 2008. Timely completion and submission of the survey allows us adequate time to complete the underwriting process and provide you with your 2009 premium:
- Please review the Instructional Manual for Users thoroughly since improvements have been made to assist you in navigating throughout the 2009 Survey. The manual is available through a link on this page or you can access it throughout the 2009 Survey by “clicking” on the field at the top and bottom of each screen which reads “Click here for complete instructions on how to review and submit the entire underwriting survey”.
- Prior to beginning the initial review of your Organization’s 2009 Renewal Underwriting Survey, it is strongly recommended that you compile all of your Organization’s data pertaining to independent living units, affordable housing units, occupied assisted living, non-skilled nursing and skilled nursing beds by county and state. This will greatly assist you in the review and completion of all questions in Section IV- Policyholder Rating Information.
- There are a few new questions contained in the 2009 Renewal Underwriting Survey. Since these questions will need to be answered for the first time no answers were transferred from the prior survey. Please take special note of these new questions; responses must be provided in order to successfully submit the 2009 Survey.
In order to complete the renewal process, the following documents are required and are to be sent to Caring Communities Shared Services, 1850 W. Winchester Road, Suite 109, Libertyville, IL 60048 Attn: Underwriting:
- Latest audited financial statement for Policyholder and its affiliates/subsidiaries
- Current listing of all owned or leased vehicles including a description of usage and the city and state where the vehicle is located
- If applicable, a list of all buildings used to house patients/residents at any of your Facilities. This listing should include street address as well as county and state.
- If applicable, a list of all owned commercial or residential buildings that are leased to others. This listing should also include street address, county and state.
- Complete copies of all contractual agreements involving medical or professional services entered into by your Organization or any of its subsidiaries/affiliates since the 2008 policy renewal
- Listing of all Facilities that are managed by your Organization or any of its subsidiaries/affiliates whether or not they are insured with Caring Communities.
- Copies of all current management agreements for all Facilities managed by your Organization or owned Facilities managed by others.
